Northwest Colorado Health, a Community Health Center located in the Grand Valley, was recently recognized as the best in class by the Community Health Provider Alliance (CHPA) for 2023. This nonprofit organization honors Community Health Centers that stand out among their peers or demonstrate significant improvement in various performance measures.
Out of 20 member Community Health Centers statewide, Northwest Colorado Health was the only health care provider to meet all targets in every category, as reported by the CHPA. The performance measures evaluated by the CHPA are designed to enhance the quality, efficiency, and coordination of patient care throughout Colorado.
