Exploring the Link Between Social Determinants and Cardiovascular Health: A Webinar by the Ohio Cardiovascular and Diabetes Health Collaborative

The Ohio Cardiovascular and Diabetes Health Collaborative (Cardi-OH) is hosting a webinar titled “Social Determinants of Health: Implementing Screening and Taking Action” on Wednesday, May 22 from noon to 1 p.m. This event aims to provide insights into the barriers and facilitators to screening for and making referrals to address social determinants of health in primary care settings, as well as current evidence on strategies to decrease the health impacts of social risk.

The keynote speaker for this webinar will be Rachel Gold from Kaiser Permanente Center for Health Research and OCHIN. As an expert in her field, she will discuss various topics related to social determinants of health and their impact on cardiovascular and diabetes health outcomes.

Those who attend this informative webinar will receive one continuing medical education credit at no cost. Cardi-OH is a statewide program with investigators located across Ohio’s medical schools, based within the Center for Clinical Investigation under Community Health Programs at Case Western Reserve University School of Medicine.
