The Community Health Needs Assessment (CHNA) Survey is being conducted by the Florida Department of Health in Pasco County (DOH-Pasco) to gather input from the community on health needs. This survey, which is available in English and Spanish, collects data on nutrition, physical activity, access to care, and mental health from Pasco County residents. The survey is open until April 15, 2024 and can be accessed at bit.ly/49jcn1t or by scanning the QR code.
The data collected from the CHNA survey will be used to develop a community health improvement plan based on the needs of the residents. To participate in the survey, you must be 18 years or older and live in Pasco County. The survey consists of 16 main questions, with additional follow-up questions based on your responses, as well as demographic questions. It is estimated to take around 10 minutes to complete.
