If you need to make changes to your medical record, it’s best done through your MyWellSpan account by navigating to the Document Center and selecting Requested Records & Record Amendments. Submit a request for a change in your medical record with detailed information. Alternatively, you can use an online or paper form like Patient Requested Amendment Form to mail or email your request for a change in your medical record.

In addition, WellSpan Health offers other medical record forms for specific requests such as Next of Kin Verification Form for deceased loved ones or 21st Century Cures Act Request Form for requests related electronic health information access, exchange or use. These forms should be used appropriately based on their purpose.
